ARE YOUR SERVICES COVERED BY MY INSURANCE?

Foundational Health and Wellness does not accept any form of commercial medical insurance or Medicare/Medicaid. However, our office can provide you with an itemized bill which can be filed for possible out-of-network reimbursement from your insurance provider. You may be able to receive partial reimbursement by filing the paperwork with your insurance provider depending upon your individual policy design and deductible. Foundational Health and Wellness does not offer any additional support in working with your insurance provider, beyond providing your itemized bill. You may use your Health Savings Account (HSA) or Flex Savings Account (FSA) to pay for visits or lab fees.

IS LAB TESTING INCLUDED WITH THE VISIT?

No, testing services are not included with the visit. However, conventional labs (LabCorp, Quest) can bill your insurance directly. We encourage you to contact your insurance company to insure you are utilizing an in-network lab and fully understand your deductible and co-payment requirements. Foundational Health and Wellness has negotiated discounted cash rates with several other lab companies should you have a significant yearly deductible. We also offer advanced testing in all therapeutic areas and feel that the integration of these lab test results makes a significant difference in optimizing your health. Functional medicine lab tests are mostly cash pay. However, these costs are well known and will be discussed during your office visit, in order to inform you of what to expect.

WHAT IS YOUR NEW PATIENT PROCEDURE?

Once your visit has been confirmed, Foundational Health and Wellness will forward an intake questionnaire and consent forms. Please take your time to fill out this important information, as it will be utilized maximize your time with us. We request these items be sent in at least 3 days prior to your visit to allow proper preparation for your visit. Any recent lab test results you would like us to review for the visit also need to be submitted at least 3 days prior to your appointment. Also include a detailed list of the medications and supplements you are currently taking. Most importantly, come to the visit ready to be engaged. We wish to partner with and empower you to make the necessary changes to maximize your health and wellness.

CAN YOU BE MY PRIMARY CARE PROVIDER?

No, Foundational Health and Wellness will not be your primary care provider. We can address most of your primary care needs, however, you are required to maintain a Primary Care Provider for routine screening exams and same-day, acute or emergent concerns that require immediate evaluation and care. Foundational Health and Wellness will work with your Primary Care Provider to support your health care needs.

WHAT IS YOUR CANCELLATION POLICY?

Communication is key to our consulting relationship. If you need to cancel an appointments, please contact our office as soon as possible. Should you need to cancel your appointment within 48 hours of its scheduled time, a $150 missed appointment fee will be charged. Thank you in advance for your consideration of my services and other clients.

HOW MANY VISITS WILL I NEED?

Your initial appointments will be 75 minutes of face-to-face time and includes considerable before and after time spent reviewing all paperwork, including the new client intake package. Most patients will require 3 to 5 follow up visits (45 minutes in duration) depending on their individual health concerns. Foundational Health and Wellness will review in depth lab results, discuss progress and modify your health plan as needed to achieve optimal and long lasting results. Follow up visits can be done in-person or via tele-health.
